Understanding Fundal Height

What is Fundal Height?

Fundal height is the distance measured from the top of the pubic bone to the top of the uterus (fundus). It's a simple, non-invasive way to monitor fetal growth and development during pregnancy.

Clinical Significance:

  • • Monitors fetal growth progression
  • • Screens for growth abnormalities
  • • Estimates gestational age
  • • Detects multiple pregnancies
  • • Identifies polyhydramnios/oligohydramnios

When to Measure

Fundal height measurements typically begin around 12-14 weeks of pregnancy when the uterus becomes palpable above the pubic bone. Measurements continue throughout pregnancy at each prenatal visit.

Factors Affecting Accuracy:

  • • Maternal obesity or body habitus
  • • Fetal position and presentation
  • • Amniotic fluid volume
  • • Multiple pregnancies
  • • Uterine fibroids
  • • Maternal bladder fullness

Clinical Guidelines

Normal Findings

  • • Fundal height in cm ≈ gestational age in weeks (±2-3 cm)
  • • Steady, consistent growth pattern
  • • 10th-90th percentile range
  • • Appropriate for gestational age

Concerning Findings

  • • <3rd percentile (growth restriction)
  • • >97th percentile (macrosomia)
  • • Plateau or decrease in measurements
  • • >3 cm discrepancy from expected
