Understanding Fundal Height
What is Fundal Height?
Fundal height is the distance measured from the top of the pubic bone to the top of the uterus (fundus). It's a simple, non-invasive way to monitor fetal growth and development during pregnancy.
Clinical Significance:
- • Monitors fetal growth progression
- • Screens for growth abnormalities
- • Estimates gestational age
- • Detects multiple pregnancies
- • Identifies polyhydramnios/oligohydramnios
When to Measure
Fundal height measurements typically begin around 12-14 weeks of pregnancy when the uterus becomes palpable above the pubic bone. Measurements continue throughout pregnancy at each prenatal visit.
Factors Affecting Accuracy:
- • Maternal obesity or body habitus
- • Fetal position and presentation
- • Amniotic fluid volume
- • Multiple pregnancies
- • Uterine fibroids
- • Maternal bladder fullness
Clinical Guidelines
Normal Findings
- • Fundal height in cm ≈ gestational age in weeks (±2-3 cm)
- • Steady, consistent growth pattern
- • 10th-90th percentile range
- • Appropriate for gestational age
Concerning Findings
- • <3rd percentile (growth restriction)
- • >97th percentile (macrosomia)
- • Plateau or decrease in measurements
- • >3 cm discrepancy from expected
